Can I use my insurance to purchase the Proclaim system?
Expenses for preventing and treating dental diseases are allowable for a Flex Spending Account (FSA), Health Savings Account (HSA), or Health Reimbursement Arrangement (HRA).
For example, if a dentist diagnoses gingivitis and recommends a Proclaim, under IRS guidelines it is an eligible expense. If your insurance provider requires a medical necessity form, click here.
Please provide the form to YOUR dental professional for a signature as our scanning partners may not feel comfortable diagnosing you, with for example, gingivitis without a full exam. Once complete, the medical necessity form can be submitted to your insurance provider for reimbursement.
If your FSA/HSA/HRA card does not have enough funds, the order will not go through. We are unable to split payment methods at this time.
Refunds are allocated to the original form of payment for any returned items.
Unfortunately, if an HSA/FSA/HRA card is used at checkout but has now expired, we are unable to edit the payment method for the refund. This is entirely based off of the HSA/FSA/HRA policies, and refunds are not guaranteed if these cards are used and then expire. If you receive a refund to an expired card, you will need to reach out to your insurance company directly for refund options.
